[00:00:07] [SPEAKER_00]: Thank you so much, thank you for having me

[00:00:10] [SPEAKER_00]: I'm very excited to talk to you today because there has been so much of talk around you know

[00:00:15] [SPEAKER_00]: how to maintain your skin at different age groups and specially women during their menopause

[00:00:21] [SPEAKER_00]: and for example when they start at the onset of menstruation

[00:00:24] [SPEAKER_00]: they face a lot of issues with their skin, primary reason being the acne that comes out of the

[00:00:31] [SPEAKER_00]: heat that the body produces. So let me start by asking you on that issue you know that

[00:00:36] [SPEAKER_00]: why is it that women start getting more acne at a relatively earlier age than men?

[00:00:42] [SPEAKER_02]: Let me tell you that men have more oil glands, more sebaceous glands and men get more acne than women

[00:00:49] [SPEAKER_02]: Really?

[00:00:50] [SPEAKER_06]: Yeah

[00:00:50] [SPEAKER_06]: Wow

[00:00:51] [SPEAKER_06]: That's the truth

[00:00:52] [SPEAKER_02]: But yes you're right during puberty all genders in fact will get acne if you have a genetic predisposition

[00:01:02] [SPEAKER_02]: If you don't have?

[00:01:03] [SPEAKER_02]: So if it is there in your genes and if you have very oily skin then during your puberty

[00:01:09] [SPEAKER_02]: you know there are changes in the hormones and that will lead to more acne

[00:01:15] [SPEAKER_02]: So but if you don't have it in the genes and if you have dry skin then you will not get acne

[00:01:23] [SPEAKER_02]: Yeah

[00:01:23] [SPEAKER_02]: So everybody need not necessarily get acne, I never got acne in my life

[00:01:28] [SPEAKER_02]: Yes

[00:01:28] [SPEAKER_02]: That's true

[00:01:29] [SPEAKER_02]: But sometimes now at this age because I'm perimenopausal I am breaking out like I have one pimple there

[00:01:37] [SPEAKER_02]: but otherwise I never earlier got acne in my life so that was because it's in my genes

[00:01:42] [SPEAKER_02]: my parents never had it, my grandparents never had it

[00:01:45] [SPEAKER_02]: probably my great grandparents never had it

[00:01:48] [SPEAKER_02]: So you have to have a few genes in you, your skin has to be oily for you to get acne at that age

[00:01:55] [SPEAKER_02]: and the hormones are just about setting up they're all lost and they're trying to find their balance

[00:02:01] [SPEAKER_02]: and that's when your estrogen-pridgeston levels are trying to set a particular level

[00:02:09] [SPEAKER_02]: There's something called the HPA axis, they're trying to reach that

[00:02:14] [SPEAKER_02]: During that period your oil glands or sebaceous glands get signals because of these hormones

[00:02:20] [SPEAKER_02]: to produce more oil then there is a normal bacteria called QT bacterium acne

[00:02:29] [SPEAKER_02]: It's normally present in the oil gland but in certain numbers

[00:02:33] [SPEAKER_02]: These oil glands receive signals and there is excessive oil or sebum production

[00:02:38] [SPEAKER_02]: These bacteria have excessive food, they are partying, they have a feast

[00:02:43] [SPEAKER_02]: So they start feeding on this and there's a multiplication and so you get pimples

[00:02:50] [SPEAKER_02]: because there is an excess of this bacteria

[00:02:53] [SPEAKER_02]: When there is an excessive oil and it's not because there is so much

[00:02:57] [SPEAKER_02]: and your pores are unable to excrete them out

[00:03:01] [SPEAKER_02]: the pores get clogged and you get whiteheads

[00:03:04] [SPEAKER_02]: When these whiteheads get exposed to sunlight they get oxidized and they become blackheads

[00:03:10] [SPEAKER_02]: So blackheads are not dirt, it's whiteheads which get oxidized and become blackheads

[00:03:17] [SPEAKER_02]: So all these changes happen around that time

[00:03:20] [SPEAKER_02]: which is why it's so common and you have to make sure at that time

[00:03:24] [SPEAKER_02]: that your lifestyle is holistic, it's very difficult

[00:03:28] [SPEAKER_02]: Imagine I'm telling a teenager don't have chocolates, don't have burgers

[00:03:32] [SPEAKER_02]: My heart is like oh my god why am I telling this kid this but I have to

[00:03:36] [SPEAKER_02]: Because it's either acne or pizzas and burgers

[00:03:41] [SPEAKER_02]: Right you have to choose

[00:03:43] [SPEAKER_02]: What lifestyle should a teenager adopt to avoid acne?

[00:03:47] [SPEAKER_02]: So what happens is if you are getting acne

[00:03:49] [SPEAKER_02]: You have to avoid food with high glycemic index

[00:03:53] [SPEAKER_02]: So pizzas, pasta, burgers, anything which has mehda in it

[00:03:58] [SPEAKER_02]: Veet pasta is fine

[00:03:59] [SPEAKER_02]: Anything in moderation

[00:04:01] [SPEAKER_02]: Anything which has sugar in it

[00:04:04] [SPEAKER_02]: Now when I say sugar I mean jaggery, honey, dates

[00:04:09] [SPEAKER_02]: Everything sugar will increase your hormone called IGF-1

[00:04:14] [SPEAKER_02]: and that will give certain signals to the oil glands to produce more oil

[00:04:19] [SPEAKER_02]: So again if you have milk

[00:04:22] [SPEAKER_02]: Milk by itself has whey, casein

[00:04:25] [SPEAKER_02]: which is anyway going to increase your androgen levels

[00:04:28] [SPEAKER_02]: the male hormone levels in the body even in a female

[00:04:31] [SPEAKER_02]: and it's also going to increase the IGF-1

[00:04:34] [SPEAKER_02]: So these are the culprits

[00:04:36] [SPEAKER_02]: And milk also sometimes you don't know if the cows are fed with a lot of antibiotics or hormones

[00:04:43] [SPEAKER_02]: and sometimes they're pregnant cows so any way the milk is rich in hormones

[00:04:47] [SPEAKER_02]: All of that is being passed on

[00:04:49] [SPEAKER_02]: And again it's a cycle where it passes on, gives signals to the oil glands

[00:04:54] [SPEAKER_02]: and you get more oil and then you get more acne

[00:04:57] [SPEAKER_02]: So milk, you can have yogurt and you can have buttermilk in moderation

[00:05:04] [SPEAKER_02]: but milk per se, sugar, mehda, potatoes

[00:05:09] [SPEAKER_02]: Kids at this age love fried potatoes

[00:05:12] [SPEAKER_02]: High glycemic index basically

[00:05:15] [SPEAKER_02]: So the good carbs you can have

[00:05:17] [SPEAKER_02]: Which are the good carbs?

[00:05:20] [SPEAKER_02]: All the vegetables, lentils, spulces, they're all good carbs

[00:05:24] [SPEAKER_02]: So you can have that

[00:05:25] [SPEAKER_02]: You can have some of the fruits

[00:05:29] [SPEAKER_02]: Don't have very sugary fruits all the time

[00:05:31] [SPEAKER_02]: Have them in moderation

[00:05:32] [SPEAKER_02]: So if somebody asks me, Doc can I take a mango as a please eat mango

[00:05:36] [SPEAKER_02]: A mango has a lot of vitamins in it

[00:05:38] [SPEAKER_02]: Vitamin A, beta carotene, antioxidants, very good for you

[00:05:41] [SPEAKER_02]: But if you have acne have it in moderation

[00:05:43] [SPEAKER_00]: Because it produces heat

[00:05:47] [SPEAKER_02]: So again it will increase your IGF 1 level

[00:05:50] [SPEAKER_02]: More than the heat, it's about the hormone

[00:05:54] [SPEAKER_02]: So we've talked about food

[00:05:56] [SPEAKER_02]: The second thing is your sleep cycle

[00:05:58] [SPEAKER_02]: So for the hormones to balance

[00:06:00] [SPEAKER_02]: The hormones need a certain circadian rhythm

[00:06:03] [SPEAKER_02]: They follow that rhythm

[00:06:05] [SPEAKER_02]: So if you're sleeping, you need to sleep early

[00:06:08] [SPEAKER_02]: And if you're sleeping at say 10, 30, 11 or 11, 30 today

[00:06:12] [SPEAKER_02]: You should follow that cycle everyday

[00:06:14] [SPEAKER_02]: You can't sleep at 11 today, one tomorrow, 3 day after

[00:06:18] [SPEAKER_02]: And say, but I'm getting my 8 hours of sleep

[00:06:20] [SPEAKER_02]: That doesn't work

[00:06:22] [SPEAKER_02]: So you need to follow that cycle

[00:06:25] [SPEAKER_02]: Sometimes when I tell people please sleep by 10, 30

[00:06:30] [SPEAKER_02]: They say it's not practical, we have homeworks

[00:06:32] [SPEAKER_02]: We have whatever things to do

[00:06:34] [SPEAKER_02]: So I say okay fine at least midnight

[00:06:36] [SPEAKER_02]: Then you make it midnight everyday

[00:06:37] [SPEAKER_02]: So that is important and get your 8 hours of sleep

[00:06:41] [SPEAKER_02]: At that age, teenage

[00:06:43] [SPEAKER_02]: You need your 8 hours of sleep

[00:06:45] [SPEAKER_02]: As you grow older, you can do with 6 hours

[00:06:48] [SPEAKER_02]: But how does that change?

[00:06:51] [SPEAKER_02]: As you grow older, you can sleep lesser

[00:06:54] [SPEAKER_02]: So what happens is your body begins to function in a way

[00:06:58] [SPEAKER_02]: That your hormones you require only that much melatonin

[00:07:01] [SPEAKER_02]: You still need rest

[00:07:03] [SPEAKER_02]: You need 6 hours minimum

[00:07:04] [SPEAKER_02]: I know people who can do with 4 hours also

[00:07:07] [SPEAKER_02]: But sleep is very essential for the hormones to balance

[00:07:10] [SPEAKER_02]: Then comes exercise

[00:07:13] [SPEAKER_02]: So even if you run or cycle or skip

[00:07:16] [SPEAKER_02]: Or whatever you want to do, dance, zumba, anything

[00:07:20] [SPEAKER_02]: But at least for half an hour, 4 days in a week

[00:07:24] [SPEAKER_02]: Is very important for the skin to stay healthy

[00:07:27] [SPEAKER_02]: So that is there

[00:07:29] [SPEAKER_02]: Then comes the products that you apply

[00:07:32] [SPEAKER_02]: These days with Instagram and a lot of other social media

[00:07:35] [SPEAKER_02]: And so much awareness

[00:07:36] [SPEAKER_02]: Kids are using a lot of wrong products

[00:07:39] [SPEAKER_02]: And a lot of products

[00:07:41] [SPEAKER_02]: Like every possible serum that they have read

[00:07:43] [SPEAKER_02]: They want to use on their face

[00:07:45] [SPEAKER_02]: And that will again clog your pores

[00:07:47] [SPEAKER_02]: And give you acne

[00:07:49] [SPEAKER_02]: So what kind of skincare routine should any youngster follow?

[00:07:53] [SPEAKER_02]: So if you are talking about a teenager

[00:07:54] [SPEAKER_02]: I will say use a good cleanser

[00:07:57] [SPEAKER_02]: That is meant for your skin type

[00:07:59] [SPEAKER_02]: So if you have oily skin, use salicylic acid based cleanser

[00:08:03] [SPEAKER_02]: At least once a day

[00:08:04] [SPEAKER_02]: Again if you have acne prone skin

[00:08:06] [SPEAKER_02]: Salicylic acid or glycolic acid or tea tree oil

[00:08:09] [SPEAKER_02]: If you have dry skin, use a cream based cleanser

[00:08:12] [SPEAKER_02]: If you have normal skin, you are lucky

[00:08:14] [SPEAKER_02]: You just use any cleanser

[00:08:15] [SPEAKER_02]: So cleanse your face or wash your face twice a day

[00:08:20] [SPEAKER_02]: Moisturize in the morning

[00:08:21] [SPEAKER_02]: And apply a sunscreen

[00:08:24] [SPEAKER_02]: Sunscreen is mandatory

[00:08:28] [SPEAKER_02]: Whether you are indoors or outdoors

[00:08:29] [SPEAKER_02]: Imagine for you this light all the time

[00:08:31] [SPEAKER_02]: For you, you need a sunscreen

[00:08:33] [SPEAKER_02]: I have not put

[00:08:34] [SPEAKER_02]: You need to

[00:08:35] [SPEAKER_02]: So it is very essential that you do this

[00:08:38] [SPEAKER_02]: Basic, just follow these three steps in the morning

[00:08:41] [SPEAKER_02]: And these three steps have been there forever

[00:08:44] [SPEAKER_02]: Cleansing, moisturizing, sunscreen

[00:08:46] [SPEAKER_02]: Where did the serum come from?

[00:08:48] [SPEAKER_02]: After that, the toner came in between

[00:08:51] [SPEAKER_02]: So then they said CTM, CTM, CTM

[00:08:56] [SPEAKER_02]: And I said no, no CTM

[00:08:58] [SPEAKER_02]: CMS

[00:09:00] [SPEAKER_02]: Right, cleansing, moisturizing, sunscreen

[00:09:03] [SPEAKER_02]: Not cleansing, toning, moisturizing

[00:09:05] [SPEAKER_02]: So that is what you need to do

[00:09:08] [SPEAKER_02]: Toning is very optional

[00:09:10] [SPEAKER_02]: Toner need not be used

[00:09:12] [SPEAKER_02]: I have never used a toner in my life

[00:09:14] [SPEAKER_02]: If you have very oily skin

[00:09:16] [SPEAKER_02]: You may use a toner

[00:09:18] [SPEAKER_02]: So that adds is like a double cleansing

[00:09:21] [SPEAKER_02]: Otherwise it's optional

[00:09:23] [SPEAKER_02]: And then came the serums

[00:09:25] [SPEAKER_02]: Now

[00:09:28] [SPEAKER_02]: The ingredients that we are talking about

[00:09:30] [SPEAKER_02]: Whether we are saying retinol

[00:09:31] [SPEAKER_02]: Retinol was discovered in 1947

[00:09:34] [SPEAKER_02]: It's been there since then

[00:09:36] [SPEAKER_02]: And we as dermatologists have been prescribing

[00:09:38] [SPEAKER_02]: Retinol forever

[00:09:40] [SPEAKER_02]: For acne, for anti-aging, for everything

[00:09:43] [SPEAKER_02]: So that's been there

[00:09:44] [SPEAKER_02]: Vitamin C was also discovered in the 80s

[00:09:47] [SPEAKER_02]: Niacinamide has been there since

[00:09:49] [SPEAKER_02]: I don't know when

[00:09:51] [SPEAKER_02]: One of the most popular brands in the country

[00:09:54] [SPEAKER_02]: The Fairness brand has Niacinamide in it

[00:09:56] [SPEAKER_02]: And our grandmothers have used it

[00:09:59] [SPEAKER_02]: So that's been there

[00:10:00] [SPEAKER_02]: You know, peptides

[00:10:02] [SPEAKER_02]: I gave my first talk on what's new

[00:10:04] [SPEAKER_02]: In Cosmos Uticles in 2011

[00:10:06] [SPEAKER_02]: When I talked about peptides

[00:10:08] [SPEAKER_02]: We're in 2020, 4 now

[00:10:10] [SPEAKER_02]: And suddenly everybody is saying peptides

[00:10:12] [SPEAKER_02]: So these ingredients have been there

[00:10:15] [SPEAKER_02]: But they've not been spoken about

[00:10:17] [SPEAKER_02]: So this I think came about

[00:10:20] [SPEAKER_02]: In the lockdown

[00:10:21] [SPEAKER_02]: Where everybody was free

[00:10:23] [SPEAKER_02]: And suddenly some wise people decided

[00:10:25] [SPEAKER_02]: To talk about skincare

[00:10:26] [SPEAKER_02]: And then they started talking about

[00:10:28] [SPEAKER_02]: Ingredients in skincare

[00:10:30] [SPEAKER_02]: And suddenly these became the buzzwords

[00:10:33] [SPEAKER_02]: And then the Cosmos Uticle

[00:10:35] [SPEAKER_02]: You know, the pharma industries

[00:10:37] [SPEAKER_02]: And the cosmetic industries

[00:10:38] [SPEAKER_02]: The beauty industries took it up

[00:10:40] [SPEAKER_02]: And then so many of these

[00:10:43] [SPEAKER_02]: Small scale and newer

[00:10:45] [SPEAKER_02]: Industries came about

[00:10:46] [SPEAKER_02]: And started making all these serums

[00:10:48] [SPEAKER_02]: And then you have skin fluences

[00:10:50] [SPEAKER_02]: And influencers and beauty bloggers

[00:10:53] [SPEAKER_02]: And all of them who've talked about it

[00:10:54] [SPEAKER_02]: So which is a good thing

[00:10:56] [SPEAKER_02]: In a way because there's so much awareness

[00:10:59] [SPEAKER_02]: And people at least know

[00:11:01] [SPEAKER_02]: That yes, skincare is important

[00:11:02] [SPEAKER_02]: But it's also a bad thing

[00:11:04] [SPEAKER_02]: Because you're using the wrong products

[00:11:06] [SPEAKER_02]: Without consulting a dermatologist

[00:11:08] [SPEAKER_02]: At least once you must consult a dermatologist

[00:11:11] [SPEAKER_00]: Yeah, exactly

[00:11:12] [SPEAKER_00]: Because there are so many products

[00:11:14] [SPEAKER_00]: Like vitamin E, A, C

[00:11:16] [SPEAKER_00]: Like how do you know which one

[00:11:18] [SPEAKER_00]: To pick up from the shop, you know

[00:11:20] [SPEAKER_00]: There are so many options today

[00:11:21] [SPEAKER_00]: So there are options and then you also have

[00:11:24] [SPEAKER_02]: You know, a lot of information

[00:11:27] [SPEAKER_02]: Right?

[00:11:28] [SPEAKER_02]: So I've seen people who just

[00:11:30] [SPEAKER_02]: Read, follow influencers

[00:11:31] [SPEAKER_02]: On social media and then

[00:11:33] [SPEAKER_02]: Pick up so many products

[00:11:36] [SPEAKER_02]: Somebody's talking about snail musin

[00:11:37] [SPEAKER_02]: The whole world is using snail musin

[00:11:40] [SPEAKER_02]: Whether it's good for you or not

[00:11:42] [SPEAKER_02]: Now it can give you allergies

[00:11:43] [SPEAKER_02]: If you have sensitive skin

[00:11:45] [SPEAKER_02]: If you're allergic to shellfish

[00:11:46] [SPEAKER_02]: It can give you allergies

[00:11:47] [SPEAKER_02]: If you have acne or acne prone skin

[00:11:50] [SPEAKER_02]: It can give you more acne

[00:11:51] [SPEAKER_02]: Alright, it has certain enzymes

[00:11:53] [SPEAKER_02]: Which will cause more acne

[00:11:56] [SPEAKER_02]: So it's not for everybody

[00:11:58] [SPEAKER_02]: But everybody uses it

[00:12:00] [SPEAKER_02]: Snail musin has become the new

[00:12:02] [SPEAKER_02]: Holy Grail for moisturizing and

[00:12:03] [SPEAKER_02]: Having the new skin

[00:12:04] [SPEAKER_02]: So I just gave you an example of snail musin

[00:12:07] [SPEAKER_02]: But every other ingredient

[00:12:08] [SPEAKER_02]: When it's really hyped

[00:12:09] [SPEAKER_02]: People think oh I cannot live without vitamin C

[00:12:12] [SPEAKER_02]: It's not true, it's an amazing ingredient

[00:12:14] [SPEAKER_02]: Don't get me wrong, I prescribe it

[00:12:16] [SPEAKER_02]: Also to a lot of my patients

[00:12:17] [SPEAKER_02]: But not every skin type will tolerate it

[00:12:21] [SPEAKER_02]: Right?

[00:12:22] [SPEAKER_02]: So not everybody probably can use retinol

[00:12:25] [SPEAKER_02]: It's one of my favourite ingredients

[00:12:26] [SPEAKER_02]: I use only retinol

[00:12:28] [SPEAKER_02]: I don't use any other serum

[00:12:29] [SPEAKER_02]: So I use a moisturiser sunscreen

[00:12:31] [SPEAKER_02]: And at night I use a retinol

[00:12:33] [SPEAKER_02]: I don't do the AHA, BHA

[00:12:37] [SPEAKER_02]: Hyaluronic acid

[00:12:38] [SPEAKER_02]: Nacinamide, vitamin C

[00:12:39] [SPEAKER_02]: I don't do it all

[00:12:41] [SPEAKER_02]: But it's not safe in pregnancy

[00:12:43] [SPEAKER_02]: Or some people can get

[00:12:47] [SPEAKER_02]: Irritation

[00:12:47] [SPEAKER_02]: A lot of irritation

[00:12:48] [SPEAKER_02]: A lot of flaking

[00:12:50] [SPEAKER_02]: So you need to know how to use the product

[00:12:52] [SPEAKER_00]: But why is retinol not safe in pregnancy?

[00:12:56] [SPEAKER_02]: So it's a large dose of vitamin A

[00:12:58] [SPEAKER_02]: And vitamin A in such large doses

[00:13:00] [SPEAKER_02]: It's terao-genic

[00:13:02] [SPEAKER_02]: That means it has

[00:13:03] [SPEAKER_02]: When the baby is formed

[00:13:07] [SPEAKER_02]: It'll cause certain deformities

[00:13:09] [SPEAKER_02]: Oh my god

[00:13:10] [SPEAKER_02]: So you gotta be careful with retinol

[00:13:12] [SPEAKER_02]: Topical retinol not as much

[00:13:14] [SPEAKER_02]: But if you're consistently applying it

[00:13:16] [SPEAKER_02]: And especially higher concentrations

[00:13:19] [SPEAKER_02]: You've got to be careful

[00:13:20] [SPEAKER_02]: So don't use it

[00:13:21] [SPEAKER_02]: And oral retinol definitely not

[00:13:23] [SPEAKER_02]: Isotretin definitely not

[00:13:25] [SPEAKER_00]: Oh my god
